Insurance Information and Medical History

Please prepare insurance information, medical history, copies of test results and hospital visits, a list of medications (names and doses), and any other relevant information (e.g. contact information for your family doctor) prior to admission at Pediatric Surgical Associates.

Create a List

Create a list of questions before your scheduled visit so you can discuss them with your child’s physician. Remember to ask for specific instructions and the doctor’s letter that you need for your child’s school or other physicians.

Items to Bring from Home

Have your child help you pack a bag for the visit or hospital stay. Include things that will help make your child’s visit or admission comfortable. For example, a few things you can bring are:

  • Toys and books
  • A favorite blanket or pillow
  • Paper and coloring pens
  • Family and pet photos
  • A hairbrush, chapstick, and toiletries
  • Comfortable clothes, pajamas, sweaters, socks, and underwear
  • Slippers

Doctor’s Pre-Admission Instructions

To prepare your child for admission, your child’s physician at Pediatric Surgical Associates will provide instructions that must be strictly followed, such as when to stop eating or drinking and when to take or stop taking certain medications.

Additional Support

It is helpful to have another adult with you during your appointment for additional support and help.

Delays & Emergency

Sometimes we experience delays due to an unexpected emergency and your child may have to wait to be seen. We appreciate your understanding in such a situation! Your child will receive the same immediate care if he or she is in a similar emergency situation.

Schedule Follow-Up Appointment

After the appointment, take note of other questions you have and important phone numbers to call, such as our emergency line, prescription refill, or appointment numbers. It’s best to arrange for your child’s follow-up appointment before you leave and write down your new appointment dates.
